Appendix 1:
– Applicable to students who have already registered their off-campus residence locally and must submit the Assessment Form from the ward (commune, township) police of their current off-campus residence to the university.

Appendix 2:
– Applicable to students who have not yet registered temporary residence with the ward (commune, township) police of their current off-campus residence (local confirmation is required)

– Students whose permanent address is identical to their current place of residence should fill out Appendix 2, attached with a notarized photocopy of their household registration, and do not need confirmation from the local ward (commune, township) police.
